Booking Forms

Breakfast/ After Schools Clubs Booking Forms

 

To book a place in Breakfast Club, Homework Club or any of our After School Clubs you must complete the correct form below each Tuesday for the following week. The forms will be live from 8am to 8pm.

 

Money must be sent in on a Wednesday morning to secure child’s place for the following week.

 

 

Please ensure all money is sent into school in separate, labelled envelopes

With child’s name class and club attending on it.

Breakfast Club Application Form

2 - 3 Afterschools Club P1 - P3 - 2pm to 3pm.

Homework Club P4 - P7 - Monday - Thursday 3pm to 4pm
